Hospice Care Packages are designed to be a convenient way to provide for your pet based on his or her individual needs.

All packages include medical management plans tailored to each pet’s unique disease process and symptoms. Concerns to be addressed include: pain, nausea, anxiety, mobility, nutritional and hydration support, etc.

Care Packages also include regular communication initiated by the veterinary nurse. Communication can be by phone, email, or both, based on preference. The nurse will provide education and support as well as act as a liaison between the family and the doctor.

 

Palliative Care Package - $30 per month

·         Medical management plan

·         Every 2 week communication

 

Early Hospice Care Package - $30 per week

·         Medical management plan

·         1 home visit with a nurse per month

·         Weekly communication

 

Intermediate Hospice Care Package - $75 per week

·         Medical management plan

·         1 home visit with a nurse per week

·         Twice weekly communication

 

Advanced Hospice Care Package - $115 per week

·         Medical management plan

·         1 home visit with a doctor per month

·         1 home visit with a nurse per week

·         Every 1-2 day communication

Standard charges apply to all prescribed medications, supplements, and other products.

 

As terminal illness progresses, pets may need to transition to a more intensive level of care. The doctor, nurse, and family will work as a team to decide when more care is needed. Transitions between levels of care may happen at any time and charges will be pro-rated accordingly.
